West Ham had taken control with first-half goals from Jarrod Bowen and Crysencio Summerville at Stamford Bridge.
Chelsea manager Liam Rosenior made three changes at the break, and his side responded with a brilliant second-half revival.
Joao Pedro began the fightback by heading home in the 66th minute.
Marc Cucurella then equalised with a diving header in the 70th minute, setting up a grandstand finish.
Fernandez capped the remarkable turnaround by thumping in the winner two minutes into added time, marking his 150th Chelsea appearance in unforgettable fashion.
West Ham’s misery was compounded in the final seconds when defender Jean-Clair Todibo was sent off for violent conduct.
Rosenior has now won six of his seven games in charge since replacing Enzo Maresca.
This victory makes him only the fourth English manager to win his first three Premier League matches.
